Worcestershire Seconds all-rounder Alex Hepburn picked up four wickets but Surrey Seconds lower order launched a strong recovery on the second day of the four-day match at Kidderminster.
Surrey resumed on 45-1 after a rain-hit opening day and wickets fell at regular intervals, including three to Hepburn in his initial spell, as they were reduced to 145-6.
But then Conor McKerr, who took 10 wickets in a County Championship match when on loan to Derbyshire against Northamptonshire recently, led a revival.
McKerr added 110th for the seventh wicket with Sam Hall and a further 87 for the eighth wicket with Rafeh Jafri.
He was eventually run out for 124 and the same fate befell Jafri (54) as Surrey were eventually dismissed for 385 in 97.2 overs.
Hepburn finished with 4-76 from 27 overs and there was a wicket apiece for skipper Charlie Morris, Alex Simpson, Ross Whiteley and Dillon Pennington.
Worcestershire were left with 15 overs batting and openers Ollie Westbury and triallist Ryan Tongue – elder brother of Josh – advanced the total to 35.
McKerr capped off a fine day for himself by dismissing Westbury for 17 in the penultimate over to leave Tongue undefeated on 15.
